Teressa Vaughn an Advance Care Planning (ACP) Consultant for Sharp Healthcare where she assist patients with advanced illness and/or advanced age complete their advance healthcare directive, discuss healthcare/living options and navigate through the healthcare maze. She also provide community education on advance care planning, aging, end of life, the End of Life Option Act (EOLOA) and grief.
